A UK guide for 2026

If a sensitivity toothpaste helped at first and then stopped, the usual reason is that it only ever did half the job. Every mainstream formula either calms the nerve or seals the exposed dentine tubules, not both. Rule out a new problem with your dentist, then switch to a formula that does both, and give it two weeks.

Around one in three UK adults lives with sensitive teeth, and three in four say the products they have tried do not give them effective relief. You have been using a sensitivity toothpaste for months, maybe years. It helped at first. Now a cold drink still makes you flinch, or the twinge has come back for no obvious reason.

This guide explains why relief fades, what each active ingredient actually does, and what to look for when you switch.

Should I see a dentist before changing toothpaste?

Yes, if the pain is new or has changed character. A toothpaste is a slow, cumulative product. Fluoride reduces decay over months of daily use, and hydroxyapatite fills micro lesions in the enamel over time. What neither does is settle a problem that arrived overnight, because sudden pain usually has a mechanical cause such as a cracked filling. Pain triggered by heat or by sweet foods is worth mentioning to your dentist too.

The everyday sensitivity a toothpaste is built for is the slow building kind, most often caused by gum recession exposing the root surface, where thousands of tiny channels called dentine tubules run straight to the nerve. This is the hydrodynamic mechanism described by Splieth and Tachou (2013).

Why is one active ingredient not enough?

Because almost every sensitivity toothpaste on the UK market relies on a single active ingredient, and each active works in only one of two ways, while sensitivity has two halves.

Nerve calming actives. Potassium nitrate builds up potassium around the nerve endings inside the tooth and dampens the pain signal before it reaches the brain. The Cochrane review by Poulsen and colleagues (2006) supports it. It is slow, taking around two weeks of daily use, and the effect fades if you stop.

Tubule sealing actives. Stannous fluoride, arginine (Colgate’s Pro-Argin technology), calcium sodium phosphosilicate (Novamin) and hydroxyapatite all work on the tooth itself, plugging the exposed tubules so cold and touch cannot reach the nerve. They do not act on the nerve.

Here is the part most people are never told. Sensodyne Repair and Protect uses Novamin, a calcium sodium phosphosilicate glass that activates on contact with water. To keep it stable in the tube, the paste has to be made without water, and potassium nitrate cannot go into a water free formula. So Repair and Protect can seal tubules, but it cannot calm the nerve. Other Sensodyne variants do the opposite: potassium nitrate alone calms the nerve but leaves the tubules open, so missing a few days brings the sensitivity back.

That is why “my Sensodyne stopped working” is such a common complaint. It is usually not that the toothpaste changed. It is that it was only ever doing half the job.

Do hydroxyapatite toothpastes help with sensitivity?

Yes, and the evidence is good. Hydroxyapatite is the mineral your enamel is made of. It bonds to enamel, rebuilds mineral lost to acid and physically seals exposed tubules, and a 2023 systematic review of 44 trials found it reduced sensitivity by 39.5% compared with placebo, and performed 23% better than fluoride on the same measure. Georganics and a growing number of UK brands now sell hydroxyapatite formulas.

What hydroxyapatite does not do is act on the nerve. It works on the tooth: it seals and rebuilds, and relief follows from that. If you have used one and still get twinges, the likely explanation is that the surface is being repaired while the nerve underneath is still over reporting. It is the same single action limit the other ingredients have, in a different place.

What should I look for when I switch?

A nerve calming active, such as potassium nitrate at 5%, the level used in clinical studies.

A tubule sealing active, such as hydroxyapatite. Nano sized particles travel into the tubules; larger biomimetic particles form a surface layer.

Fluoride, still the best evidenced ingredient for strengthening enamel and preventing decay. Some hydroxyapatite toothpastes drop it. You do not have to choose.

No whitening agents and no SLS if your gums or mouth are easily irritated.

A stated percentage, and a brand that says whether that percentage refers to the raw material as supplied or to the active content. They are different numbers.

Give any new toothpaste at least two weeks of twice daily brushing before judging it. Use a soft brush, brush in small circular motions and do not press hard. Brushing too hard is one of the main drivers of the gum recession that causes sensitivity in the first place.

A UK formula built around this problem

S3 Daily Sensitive Toothpaste, from British brand S3 Sensitivity Science — backed by 20+ UK dentists — was developed to combine both approaches in one water based formula. Its patent pending S3 Repair Technology pairs:

potassium nitrate at 5%, to calm the nerve;

nano hydroxyapatite at 10% solution, to travel into the dentine tubules and seal them;

biomimetic hydroxyapatite at 5% solution, to form a protective mineral layer on the surface;

fluoride at 1450 ppm as sodium monofluorophosphate, a stable salt that does not react with the calcium in the formula.

The formula was developed over three years and 31 iterations by Majedeh Mazraei, a formulation scientist who previously worked on Sensodyne and Aquafresh at GSK. In independent testing at the University of Reading it deposited around three times more calcium and phosphorus on the dentine surface than Sensodyne Repair and Protect, and achieved 100% dentine tubule coverage within 24 hours. In a consumer study of 52 existing sensitive toothpaste users, 88% reported an improvement within seven days.

It is SLS free and vegan, costs £19.99 a tube or £14.99 on subscription, and comes with a 21 day money back promise.

The takeaway

If your sensitivity toothpaste has stopped working, the most likely explanation is that it only ever addressed one part of the problem. Check with your dentist that nothing new is going on, then switch to a formula that calms the nerve and seals the tubules at the same time, and give it two weeks.

Frequently asked questions

How long before a new sensitivity toothpaste works?

Give it two weeks of brushing twice a day. Potassium nitrate needs around two weeks to build up around the nerve endings, so judging a formula after a few days tells you very little.

Can a toothpaste fix sudden tooth pain?

No. Sudden pain, or pain on heat and sweet foods, usually has a mechanical cause such as a cracked filling, and needs a dentist. Toothpaste works slowly and cumulatively on everyday sensitivity.

Why does sensitivity come back when I stop using the toothpaste?

With a nerve calming active such as potassium nitrate the effect fades once you stop, because it depends on potassium staying around the nerve endings. A formula that also seals the tubules leaves a physical barrier behind, which is why combining the two matters.

Is hydroxyapatite better than fluoride for sensitive teeth?

They do different jobs. Hydroxyapatite rebuilds mineral and seals tubules, fluoride makes enamel more acid resistant and prevents decay. For sensitivity you also want an active that calms the nerve, which neither of them does.

     <li class="share_item
