These third molars often lack room to come in properly, leading to impaction, crowding, infection or damage to neighboring teeth.

The surgery itself is usually quick. Recovery takes a little longer, and knowing what is normal and what is not makes it much easier to get through. This guide covers why wisdom teeth are removed, a typical recovery timeline, how to reduce your risk of dry socket, and the signs that mean you should call your surgeon.

Why Wisdom Teeth Are Removed

Wisdom teeth usually appear in the late teens or early twenties, long after the rest of the adult teeth. By then, there is often not enough room at the back of the jaw. The teeth may remain trapped under the gum or bone (impacted), come in at an angle, or only partially break through.

Partially erupted wisdom teeth create pockets that trap food and bacteria, which can lead to painful gum infections. Impacted teeth can damage the roots of neighboring molars, and in less common cases cysts can form around them. Because these teeth are so far back, they are also difficult to clean and prone to decay.

Not every wisdom tooth needs to come out. Teeth that are fully erupted, healthy, functional and easy to clean may be monitored instead. Your surgeon will review X-rays and discuss whether removal is recommended now, later, or at all.

Before Surgery: Setting Yourself Up for Success

Preparation makes a real difference. Stock up on soft foods, arrange for someone to drive you home if you are having sedation, fill any prescriptions in advance, and plan to rest for at least a couple of days. Follow your surgeon’s instructions about eating and drinking before the procedure.

Every surgical team provides tailored aftercare advice, and those instructions should always come first. Oral surgery practices such as Peachtree Oral & Facial Surgery typically walk patients through recovery before the day of surgery, so you know what to expect and who to contact if you have concerns.

Recovery Timeline: What to Expect

The first 24 hours. Some bleeding is normal. Bite gently on gauze as directed and replace it as needed. Rest with your head elevated. Apply ice packs to the outside of your cheeks in cycles of about 20 minutes on and 20 minutes off to limit swelling. Stick to cool, soft foods and fluids. Do not spit, rinse vigorously, smoke or use straws, as the suction can dislodge the blood clot forming in the socket.

Days 2 to 3. Swelling and stiffness usually peak during this window. Some bruising on the cheeks or jaw is common. Continue pain relief as directed. After the first day, your surgeon may advise gentle warm salt water rinses, particularly after meals.

Days 3 to 5. Swelling should start to settle and you should feel noticeably better. This is also the period when dry socket most often appears, so pay attention to any pain that worsens rather than improves.

Days 5 to 7. Most people return to normal activities by the end of the first week. You can gradually reintroduce more textured foods, avoiding chewing directly on the surgical sites. Any stitches may dissolve or be removed at a follow-up visit.

Weeks 2 and beyond. The sockets continue to fill in with new tissue over several weeks, and the bone underneath takes months to fully remodel. Keep the area clean and follow up as advised.

Managing Pain and Swelling

Most discomfort can be controlled well with the medication plan your surgeon provides. Take pain relief on schedule for the first day or two rather than waiting until pain builds, as it is easier to stay ahead of discomfort than to catch up. Follow dosage instructions exactly and do not combine medications without checking with your surgeon or pharmacist.

Cold packs work best in the first 24 to 48 hours. After that, some surgeons recommend switching to gentle warmth to help with jaw stiffness. Keeping your head elevated when resting, staying hydrated and avoiding strenuous exercise for a few days all help swelling resolve faster. Jaw stiffness is common and usually improves with time and gentle opening and closing exercises once your surgeon approves.

Understanding Dry Socket

Dry socket, known clinically as alveolar osteitis, happens when the blood clot in the extraction site is lost or breaks down too early, exposing the underlying bone. It is the most common complication after wisdom tooth removal.

Research shows the risk varies depending on the type of extraction and personal habits. One study of more than 1,300 extractions found an overall dry socket rate of 3.2%, rising from 1.7% after simple extractions to 15% after surgical extractions. The same study found that dry socket was significantly more common in smokers (12%) than non-smokers (4%), with a strong link to the amount smoked. Reviews also note it occurs more often in the lower jaw, especially after removal of lower third molars, with trauma, poor oral hygiene and smoking increasing the risk.

Signs of dry socket include:

Pain that gets worse instead of better, typically starting a few days after surgery

Pain radiating to the ear, eye or temple on the same side

A visibly empty socket or exposed bone

Bad breath or an unpleasant taste

Dry socket is painful but treatable. Your surgeon can clean the area and place a medicated dressing, which often brings rapid relief.

How to Lower Your Risk of Dry Socket

Avoid smoking and vaping before and after surgery for as long as your surgeon advises

Do not use straws or spit forcefully for the first several days

Avoid vigorous rinsing in the first 24 hours

Keep the mouth clean with gentle brushing and rinses as directed

Follow all medication instructions, including any antiseptic rinse

What to Eat During Recovery

In the first few days, choose foods that need little or no chewing: yogurt, smoothies (eaten with a spoon), mashed potatoes, soups that have cooled to lukewarm, scrambled eggs, applesauce and oatmeal. Avoid crunchy, spicy, very hot or small, hard foods like seeds and nuts that can lodge in the sockets. Stay well hydrated.

When to Call Your Surgeon

Contact your surgeon promptly if you experience:

Bleeding that does not slow after firm pressure with gauze

Pain that worsens after day 3 or is not controlled by prescribed medication

Swelling that increases after the third day

Fever, chills or pus from the socket

Numbness of the lip, chin or tongue that persists after the anesthetic should have worn off

A persistent bad taste or smell

Seek emergency care immediately for difficulty breathing or swallowing.

The Bottom Line

Most people recover from wisdom teeth removal smoothly within a week or so. Good preparation, careful aftercare and avoiding smoking and straws all lower the risk of complications. Knowing what normal healing looks like, and when to pick up the phone, helps you get back to everyday life with confidence.

     <li
